Terms and conditions 

 

A 3-month minimum commitment is required for all membership packages.

If you cancel before, you will be charged a $200 cancelation fee for a 1 IV per month package.

2 IVs per month will be a $400 cancelation fee

3 IVs per month will be a $600 cancelation fee

4 IVs per month will be a $800 cancelation fee

If you don't use your monthly IV that month, it rolls over, and what's even better is that you can let them add up; yes, that's right, if you don't use them for three months, you will have three waiting.

 

You can gift an IV to a friend or family member.

 

You can use your monthly infusion toward a different package than those listed and receive $175 monthly for any package you like. 

 

It can't be combined with the NAD buy 4 get 1 Free.

 

If you want to use your membership IV towards a more expensive infusion, one month will equal $175 credit applied to another infusion: NAD+, Iron, High-Dose Vitamin C 50mg, or 70mg.

 

Memberships must be canceled with a 30-day notice via email to Membership@MyVitaminTherapy.com. 

 

Your credit card will be kept on file and charged on the day of the month you originally signed up. 

 

You may only cancel the membership by emailing.
