Membership Commitment Terms and Conditions


A 3-month minimum commitment is required for all membership packages.

  • Early cancellation fees apply:

    • 1 IV per month → $200 fee

    • 2 IVs per month → $400 fee

    • 3 IVs per month → $600 fee

    • 4 IVs per month → $800 fee
       

IV Rollovers

  • Unused IVs roll over to the next month.

  • Rollovers accumulate, so if you skip three months, you’ll have three IVs waiting.
     

Sharing Benefits

  • You may gift an IV to a friend or family member.
     

Membership Usage

  • Memberships are valid for in-clinic appointments only (mobile services excluded).

  • Each monthly infusion can be applied as a $175 credit toward any other package.

  • If you’d like to upgrade your infusion (NAD+, Iron, or High-Dose Vitamin C 50mg/70mg), your monthly IV will be applied as a $175 credit toward the upgraded service.

  • Membership benefits cannot be combined with the NAD+ “Buy 4, Get 1 Free” promotion.
     

Billing & Cancellation

  • Your credit card will be securely kept on file and billed each month on your original sign-up date.

  • Membership cancellations require a 30-day written notice via email to:
    Reservations@MyVitaminTherapy.com

  • Cancellations are only accepted by email.
